What Is Fatty Liver?
Fatty liver means excessive accumulation of fat within liver cells.
The terminology has changed in recent years.
What was traditionally called NAFLD – Non-Alcoholic Fatty Liver Disease is now commonly termed MASLD – Metabolic Dysfunction-Associated Steatotic Liver Disease when liver fat occurs along with cardiometabolic risk factors.
The inflammatory form previously called NASH is now termed MASH – Metabolic Dysfunction-Associated Steatohepatitis.
For patients, however, the familiar term “fatty liver” remains widely used.
Who Is at Risk of Fatty Liver?
Fatty liver is strongly associated with metabolic problems such as:

These associations are well recognized in metabolic fatty liver disease.

Is Fatty Liver Dangerous?
This is one of the most common questions patients ask.
“Doctor, ultrasound mein Grade 1 fatty liver hai. Kya dangerous hai?”
Fatty liver does not mean that every patient will develop serious liver disease.
However, some patients can progress through:
Liver fat → inflammation → fibrosis → cirrhosis
This is why the most important question is not simply:
“Fatty liver hai ya nahi?”
It is:
“Is there significant liver inflammation or fibrosis?”
MASLD can range from simple steatosis to MASH, fibrosis and cirrhosis.
Fatty Liver Often Has No Symptoms
Many patients with fatty liver feel completely normal.
When symptoms occur, they may be nonspecific, such as:

Mild right-upper abdominal discomfort

Feeling of heaviness

Abnormal liver enzymes found incidentally
Fatty liver is commonly a silent disease.
Therefore, absence of pain does not necessarily tell us whether fibrosis is present.
What Is FibroScan?
FibroScan, or vibration-controlled transient elastography, is a non-invasive test used to assess liver stiffness.
Higher liver stiffness may indicate increasing fibrosis, although results have to be interpreted in the context of the patient's clinical condition.
Elastography is one of the tools doctors can use to estimate liver fibrosis without immediately performing a liver biopsy.
A FibroScan report should therefore not be interpreted simply as:
“Score high hai, matlab cirrhosis hai.”

What Are SGOT and SGPT?
Patients commonly receive reports showing:

However, raised liver enzymes are not a diagnosis by themselves.
