Phimosis: Causes, Symptoms and Treatment

By Dr. Karan R Rawat – General & Laparoscopic Surgeon in Agra

Phimosis is a condition in which the foreskin of the penis cannot be pulled back (retracted) over the glans. It is common in young boys but can also occur in adult men due to infection, poor hygiene, or chronic inflammation.

Although mild phimosis may not cause major problems, severe phimosis can lead to pain, infection, difficulty in urination, and sexual discomfort.

What is Phimosis?

Phimosis occurs when the foreskin becomes tight and cannot move backward over the head of the penis.

This condition involves the prepuce (foreskin) and the glans penis (tip of the penis).

There are two types:

1. Physiological Phimosis

• Normal in children
• Usually resolves naturally with age

2. Pathological Phimosis

• Occurs in adolescents or adults
• Caused by infection, scarring, or inflammation

Causes of Phimosis

Common causes include:

• Poor genital hygiene
• Recurrent infections
• Inflammation of foreskin
• Diabetes mellitus
• Forceful retraction of foreskin in childhood
• Skin diseases affecting the penis

Infections like **Balanitis (inflammation of glans) and **Balanoposthitis (inflammation of foreskin and glans) frequently contribute to phimosis.

Symptoms of Phimosis

Symptoms may include:

• Inability to retract foreskin
• Pain during erection
• Difficulty passing urine
• Swelling or redness of foreskin
• Recurrent infections
• Foul smell or discharge

In severe cases, the foreskin becomes extremely tight and painful.

Complications if Untreated

If phimosis is ignored for a long time, complications may occur such as:

• Recurrent infections
• Painful erections
• Urinary obstruction
• Increased risk of **Paraphimosis (foreskin stuck behind the glans)
• Rarely, increased risk of penile cancer

Therefore timely evaluation by a surgeon is recommended.

Diagnosis of Phimosis

Diagnosis is usually done by:

• Physical examination
• Assessment of foreskin retractability
• Screening for infection or diabetes if required

In most cases, no complex investigations are required.

Treatment of Phimosis

Treatment depends on the severity.

1. Medical Treatment

Mild cases may improve with:

• Topical steroid creams
• Antibiotics if infection present
• Proper hygiene

2. Surgical Treatment

For severe or recurrent cases, surgery is the best option.

Common procedures include:

Circumcision (removal of foreskin)
• Preputioplasty (foreskin preserving surgery)
• Laser circumcision

These procedures are safe, quick, and usually performed as day-care surgery.

Hygiene Tips to Prevent Phimosis

✔ Maintain proper genital hygiene
✔ Gently retract foreskin during bathing (in adults)
✔ Treat infections early
✔ Control diabetes
✔ Avoid forceful retraction in children
